Vidalia, GA Mileage Reimbursement: $0.43/mile Pay Rate: $56.14/hour, paid weekly

Schedule:

Monday – Friday, 8:00 AM – 5:00 PM On-call rotation: one week per month Requirements Associate’s or Bachelor’s Degree in Nursing or related field Active, unrestricted Registered Nurse (RN) license in Georgia or compact state Case Management certification preferred Minimum 1 year of home health experience – Required OASIS experience – Required Homecare Homebase knowledge – Required Clinical skills required: wound vac, PICC line care, ostomy care, IV antibiotics administration, and wound care

Key Responsibilities Conduct comprehensive assessments of high-risk members to identify care needs Collaborate with primary care providers to develop and implement individualized care plans Continuously evaluate and adjust care plans to ensure cost-effective and appropriate care levels Coordinate transitions of care with healthcare professionals and community resources Engage specialty services as needed to support optimal patient outcomes Maintain accurate and detailed documentation of clinical and functional progress Advocate for patients while ensuring privacy and confidentiality standards are upheld
