At Virginia Home Care Services, we are dedicated to providing exceptional home care services with compassion, integrity, and excellence. We’re seeking a detail-oriented and motivated Quality Assurance (QA) RN to join our team and ensure the highest standards of care.
Position OverviewThe Quality Assurance (QA) RN is responsible for ensuring clinical, regulatory, and documentation compliance across all skilled home health services. This role reviews clinical records for accuracy, completeness, and adherence to federal, state, accreditation, and payer requirements. The QA RN works closely with clinicians, intake, and leadership to identify deficiencies, provide feedback, and implement process improvements that support patient outcomes and agency compliance.
Key Responsibilities- Clinical Documentation Review
- Audit OASIS assessments, visit notes, POCs, and discharge summaries for accuracy, completeness, and regulatory compliance.
- Verify correct use of ICD-10 codes, therapy reassessments, and visit frequencies.

Education & Licensure
- Current RN in good standing in VA
Experience
- Minimum 2–3 years of clinical experience in skilled home health setting with QA experience
- Prior experience in quality assurance, compliance, clinical auditing, or OASIS review required
- Familiarity with Medicare/Medicaid guidelines, CoPs, and accrediting body standards (ACHC, CHAP, or Joint Commission)
Technical & Documentation Skills
- Strong knowledge of OASIS, Plans of Care (POC), visit documentation, and recertification processes
- Proficiency with EMR/EHR systems (HCHB) preferred
- Understanding of ICD-10 coding guidelines for home health
Regulatory & Compliance Knowledge
- Working knowledge of Conditions of Participation (CoPs), billing compliance, and documentation requirements
- Ability to identify deficiencies and ensure corrective action
