This travel assignment provides a weekly blended rate of $2,600, based on five 8‑hour day shifts. Key Responsibilities of the Case Management RN

Complete discharge planning assessments and identify resource needs
Communicate and collaborate with providers, therapy, pharmacy, and support teams
Arrange post‑acute services including home health, SNF, rehab, DME, and follow‑ups
Conduct utilization reviews and verify medical necessity
Provide education to patients and families regarding ongoing care
Maintain timely, compliant documentation reflecting care‑coordination activity
Support safe, patient‑centered care transitions

Requirements of the Case Management RN

Active West Virginia RN license (or compact eligibility)
Current BLS certification
Minimum 1–2 years of RN Case Management experience
Strong communication and problem‑solving abilities
