Key responsibilities of the RN – Case Management – Travel include:
- Perform comprehensive assessments of high-risk members.
- Collaborate with primary care providers to implement individualized care plans.

Requirements of the RN – Case Management – Travel include:
- Associates or Bachelor's Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license.
- Ability to analyze complex medical information and make decisions.
- Case Management certification.
- Experience with HH, OASIS, and HCHB.
