Licensed Practical Nurse (LPN) – Home Health
Schedule: Monday–Friday, 8:00 AM–5:00 PM + evening/weekend on-call rotation
License: WV LPN
Productivity: 32 points/week
Mileage: $0.54/mile
Counties: Harrison, Marion, Tyler, Barbour, Doddridge, Lewis & Upshur
Job Overview
We are seeking a Home Health LPN to provide quality patient care in the home while promoting dignity, independence, and safety. The LPN will follow individualized care plans, monitor patient conditions, administer medications and treatments, and coordinate with the care team.
Key Responsibilities
- Provide nursing care according to the patient's plan of care.
- Monitor vital signs and report abnormal findings or changes in condition.
- Administer medications, feedings, oxygen, ostomy care, and other treatments as ordered.
- Complete care routines established by PT, OT, Speech, and other providers.
- Maintain infection control and safety standards.
- Accurately document care, patient progress, and outcomes.
- Order necessary supplies, medications, and feedings.
- Monitor incidents, complete required reports, and notify appropriate personnel.
- Ensure medical equipment is properly maintained and report issues for repair.
Requirements
- Active WV LPN License
- Home Health experience required
- HCHB experience required
- Strong clinical, documentation, and communication skills
- Ability to meet 32 points/week and participate in evening/weekend on-call rotation
