Deliver compassionate, patient-centered care as a hospice registered nurse, supporting individuals and families during the end-of-life journey in home and residential settings. This essential nursing role involves daily field visits, forming strong clinical partnerships, and dedicating your expertise to enhance comfort and dignity for patients coping with terminal illness.

Schedule:
  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation as needed
  • Caseload of approximately 45 patient visits per day
Key Responsibilities:
  • Perform thorough nursing assessments to develop and update personalized hospice care plans.
  • Deliver skilled nursing care, focusing on symptom management, pain control, and holistic comfort.
  • Administer medications and treatments following physician directives and hospice standards.
  • Regularly monitor patient status, promptly communicating any changes to the interdisciplinary care team.
  • Provide education to patients and caregivers on disease progression, symptom relief, and emotional well-being.
  • Collaborate with social workers, chaplains, aides, and other team members to address physical, emotional, and spiritual needs.
  • Document care and patient progress accurately using Home Care Home Base (HCHB).
  • Participate in team meetings, case conferences, and quality improvement initiatives.
  • Uphold hospice policies, procedures, and all relevant regulatory guidelines.
Qualifications:
  • Current, active RN license (state of practice)
  • Minimum of 1 year recent hospice nursing experience
  • Proficiency with Home Care Home Base (HCHB)
  • Excellent assessment, communication, and interpersonal skills
  • Strong knowledge of pain management and end-of-life care principles
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • Self-directed, organized, and able to manage a mobile caseload
  • Compassionate, resilient, and devoted to patient advocacy
  • BSN preferred
  • Familiarity with Medicare hospice documentation standards is advantageous

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
