Responsibilities: > >
Provide direct patient care and skilled nursing services to home care clients in the Vineland, NJ area >
Conduct comprehensive patient assessments and develop individualized care plans >
Administer medications, wound care, IV therapy, and other clinical treatments as ordered >
Monitor and document patient progress, updating care plans as needed >
Educate patients and family members on disease management, medications, and home safety >
Collaborate with physicians, therapists, and interdisciplinary care team members >
Ensure compliance with home health regulations, documentation standards, and agency policies >
Requirements: > >
Active Registered Nurse (RN) license in the state of New Jersey >
Previous home care, home health, or community nursing experience preferred >
Strong clinical assessment and critical thinking skills >
Excellent communication and patient education abilities >
Reliable transportation and valid driver's license for home visits >
Ability to work independently and manage a patient caseload with minimal supervision >
CPR/BLS certification required >
