Wellstar Health System is seeking a Full-time Home Health RN Case Manager to provide and supervise primary care for 25–35 patients in the home setting. You will coordinate multidisciplinary care, document in EMR, and ensure patient goals are met.
The role demands strong communication with physicians, families, and care teams, with typical shifts 8:30–5, Monday through Friday. Prior post-acute/post-hospital experience is valued.
