Advanced Practice Provider - Palliative Care

Job Overview

Sentara Medical Group is hiring a full‑time Nurse Practitioner or Physician Assistant to join the Palliative Care team on the campus of Sentara Princess Anne Hospital, Virginia Beach, VA.

Work Shift:

First (Days), Monday‑Friday; 8:30 a.m. – 5:00 p.m.

Provider Specialty:
Palliative Care.

• Perform palliative care consultations and follow‑up visits for hospitalized patients with acute palliative care needs, including symptom management and complex medical decision making.
• Facilitate family meetings related to current medical decision making and advance care planning.
• Assist with developing appropriate discharge plans with respect to established goals of care, collaborating closely with Hospice for patient access and continuity of care.
• Actively participate as a member of an interdisciplinary team to ensure palliative care addresses pain and symptom management, psychosocial/spiritual support, and caregiver support.
