Inpatient Case Manager - Registered Nurse

Overview

Sentara Princess Anne Hospital is hiring a full‑time RN Case Manager for our inpatient units.

• Case Manager RN
• Full‑time; 40 hr/week
• A minimum of 12 months of nursing experience is required

• Provide care to patient and family throughout inpatient stay, building relationships.
• Act as a clinical leader of the healthcare team, driving decisions to ensure safest, effective transition plans.
• Collaborate with nurses, social workers, and other team members.
• Develop and implement comprehensive care plans addressing physical, mental, and psychosocial needs.
• Facilitate clinical appropriateness and length of stay, ensuring proper disposition to appropriate level of care.
• Promote effective utilization and monitoring of health services; communicate with team and patient/caregiver to manage care and transitions.

• Active RN license in Virginia.
• Minimum 12 months of nursing experience.
• Experience in discharge planning, case management, or similar is an asset.
• Strong critical thinking and communication skills.
