Role Overview

The Registered Nurse (RN) Case Manager provides comprehensive nursing care to hospice patients by conducting initial and ongoing assessments of the impact of a terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs. This role develops, implements, and revises individualized plans of care, provides ongoing education and support to patients and their families, and collaborates closely with the interdisciplinary hospice team.

The RN Case Manager also supervises Licensed Practical Nurses (LPNs) and Hospice Aides while consistently modeling Westminster Canterbury’s core values of respect, passion, and professionalism.

What You’ll Do In This Role
  • Assess the patient's physical, functional, psychosocial, environmental, and daily living needs, including grief risk, cultural/spiritual considerations, and communication patterns.
  • Implement and revise individualized care plans as needed, managing discomfort, providing symptom relief, and delivering specialized palliative and end‑of‑life nursing care.
  • Educate and support patients and families on the disease process, self‑care, end‑of‑life care, and ethical concerns.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.
  • Coordinate all patient/family services and prioritize needs with the interdisciplinary team, using the case management approach and referring to other services as needed.
  • Inform the physician and other personnel of changes in the patient's needs and outcomes of intervention.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Supervise LPNs and paraprofessionals providing services to the patient according to regulatory guidelines.
  • Maintain compliance and participate in in‑service and training programs as assigned.
  • Provide specialized hospice training to staff, families, and informal caregivers, with ongoing evaluation of patient and family responses to care.
  • Evaluate caregiver ability to meet patient needs and identify personal support needs and resources throughout care.
  • Apply specific criteria for admission and re‑certification to hospice care to establish appropriate levels of care and the patient's eligibility.
  • Communicate information using current processes and technology available to the organization.
  • Participate in the hospice performance improvement program.
  • Rotate on call as needed.
  • Perform other duties as requested.
Core Competencies
  • Demonstrate strong verbal and written communication skills for clear and professional interactions.
  • Effectively meet deadlines and perform responsibilities with minimal supervision.
Physical Requirements
  • Perform cardiopulmonary resuscitation and relieve an obstructed airway.
  • Climb at least 2 flights of stairs.
  • Stand and/or walk for a period of at least 4 hours.
The Experience You’ll Bring

Required:

  • At least one year of professional nursing experience.
  • Unencumbered Registered Nurse License in the state of Virginia (or compact state).
  • Current CPR certification (Basic Life Support).
  • Experience with Microsoft Office and Google Suite.
  • Motor Vehicle Driving Record satisfactory to Westminster‑Canterbury and its insurance providers.
  • Proof of motor vehicle insurance.

Preferred:

  • Experience in Hospice, Home Health, or a Skilled Nursing Facility.
  • Compensation Range: $40.00 - $43.00 per hour (based on relevant experience)
