Primary Responsibilities
- Conduct OASIS visits in the home health setting.
- Complete comprehensive patient assessments and documentation.
- Perform approximately 2 Admissions/Resumption of Care (ROC) visits daily.
- Develop and implement patient care plans.
- Coordinate care with physicians, caregivers, and interdisciplinary team members.
- Maintain accurate and timely documentation in compliance with agency standards.
- Provide skilled nursing care and patient education in the home setting.
- Active RN License
- Home Health Clinical
Experience:
Minimum 2 years preferred - Homecare Homebase (HCHB)
Experience:
Required - Strong assessment and documentation skills
- Ability to manage a home health caseload independently
