Licensed Practical Nurse (LPN)

Base rate $26–$34 per hour with an additional $0.50 shift differential for evening shifts. Sign‑on bonus of $7,500 and daily pay available through a trusted third‑party provider. • Collect required information from new residents for admission.
• Record residents’ health details, including vitals and temperature.
• Administer medications and injections as required.
• Treat and dress wounds and bedsores.
• Supervise certified nursing assistants (CNAs) as needed.
• Assist residents with dressing and personal hygiene.
• Monitor residents’ food and liquid intake and output.

• Must be able to work as a team member.
• Valid LPN state license.
• In good standing with the state registry.

Warren Center is an eighty‑bed skilled nursing facility located at 42 Gurney Lane. We provide a range of nursing care, including short‑term rehabilitation for those who cannot immediately return home and long‑term care for individuals unable to care for themselves. We address each person individually and use a team approach, working with residents and families to develop a unique plan of care.
