• 15 – 22 patients per day depending on SNF census
• Skilled nursing facility (SNF) setting
• Adults only — open to internal medicine or family medicine
• Standard primary care skilled nursing visits required
• Regulatory visits — MD responsible for every other 60-day visit
• Hospital privileges required
• Minimum 1-year locums or temp-to-perm commitment required
