Responsibilities Include But Are Not Limited To- Evaluates and treats hospice patients in a home setting.
- Communicates with families, physicians, and other health care professionals.
- Maintains daily, thorough documentation of services in the EMR.
- Has your own equipment, including smart devices and a blood pressure monitor.
- Graduate of an accredited college or university with a Doctoral, Masters, or Bachelors degree in Physical Therapy.

Experience:
New graduate applicants welcome; experienced therapists, especially those with hospice experience, are encouraged. - Actively licensed as a FL Physical Therapist.
