Home Health Occupational Therapist - per visit

Overview

BAYADA Home Health Care is seeking a Home Health Occupational Therapist (OT) for a part-time opportunity in Berryville, Virginia. This office focuses on home health visits in private homes throughout Northern Virginia. The OT will be part of a multi-disciplinary health care team delivering rehabilitative care and skilled nursing to clients in their homes.

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans.
• Evaluate home environments for hazards or barriers to independent living; identify equipment needs and environmental adaptations.
• Continually assess and revise the OT care plan and participate with nursing in the multidisciplinary care plan as appropriate.
• Educate and instruct patients, family members, or other representatives in rehabilitative care to promote health, safety and independent living.
• Accurately document observations, interventions, and evaluations using the available EMR tools.

• Current Virginia Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum of one year of recent work experience as an OT.
• Graduation from an occupational therapy program accredited by the AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
