This is a full-time, on-site role for a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker Associate (LCSWA). The professional in this role will provide direct social work services, including conducting comprehensive assessments, developing care plans, and offering mental health and behavioral health interventions. Responsibilities include case management, collaborating with healthcare providers, and ensuring patients' needs are met effectively.

The role also involves working closely with individuals and families to support their overall well-being and address unique challenges.

• Competency in Social Work and Medical Social Work practices
• Proficiency in Case Management and experience in coordinating services to meet patient needs
• Knowledge and experience in Mental Health and Behavioral Health interventions
• Excellent communication skills and ability to build trust with clients and their families
• Master's degree in Social Work (MSW) and a valid LCSW or LCSWA license
• Experience with diverse populations and sensitivity to cultural differences
• Ability to work independently and collaboratively in a healthcare setting
• Familiarity with clinical documentation and electronic health records is a plus
