Job SummaryThe primary purpose of this position is the provision of safe, effective and optimal medication therapy to patients with a broad range of disease states. The clinical pharmacist supports medical providers and serves as a member of the interdisciplinary team to address patient needs, meet quality measures and provide pharmaceutical care services to Center patients according to professional standards and state and federal legal requirements.
The Clinical pharmacist must maintain open communication with providers, pharmacy staff and members of the center’s staff. A positive, professional demeanor must be projected through verbal and non‑verbal communications.
Specific Responsibilities- Participate in resolving any medication therapy problems including therapeutic duplications, medication allergies, clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions. Makes appropriate evidence‑based, patient-centered medication recommendations.
- Communicates effectively and appropriately to provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
- Administer immunizations based on collaborative practice agreement. Maintain appropriate records and complete necessary charting of immunizations in the electronic health record.
- Participate in Medication Therapy Management, Comprehensive Medication Reviews as applicable.
- Responsible for investigating, planning, implementing and maintaining clinical pharmacy services as opportunities arise. Assure pharmacy is an integral part of the health-care delivery system and facilitate enhancement and expansion of pharmacy services and programs.
- Support Quality Management team in developing and implementing tools to close care gaps and meet quality measures.
- Participate in Interdisciplinary Team and Care Management Committee.
- Facilitates experiential education, maintains proficiency and models mentorship skills for Pharm.
D. candidates. - May participate in pharmacy operations and medication dispensing.
- Be familiar with and follow the policies and procedures of Blue Ridge Medical Center and Blue Ridge Medical Center Pharmacy.
- Graduate of an ACPE accredited College of Pharmacy.
- Doctor of Pharmacy or other advanced degree highly preferred.
- Meets the requirements of the Commonwealth of Virginia regarding licensure for practice as a Registered Pharmacist.
- Completion of pharmacy-based immunization certificate or equivalent.
- Basic Life Support CPR certification.
- Possess the knowledge and security to make effective independent decisions and adapt when circumstances warrant it.
- Provide clear and concise guidance and instructions. Possess excellent and professional writing and oral communication skills.
- Maintain the necessary confidentiality over sensitive and confidential matters of the Center and its business.
