Responsible for the implementation of a suitable individualized service plan for individuals with intellectual disabilities who reside in their own homes, in the community or other appropriate settings. Services may include in‑home training, personal assistance, in‑home respite services, and out‑of‑home respite services. Input as to needed services will result from a team approach and will include involvement from case management services, family members, and appropriate therapeutic consultants.

The purpose of this position is to enhance the individual’s level of independence in the community, and to prevent a more restrictive placement.

• Provide personal care and assistance such as bathing, dressing, grooming, feeding, cleaning the individual’s room, washing and drying laundry, and preparing meals.
• Teach skills to increase independence.
• Assist in medication administration, replenishment, repackaging, and documentation.
• Transport individuals on community outings and doctor’s appointments to increase social skills.
• Serve as a member of the planning team, working closely with assistant supervisors, case managers and other suitable professionals to ensure goals are met.
• Maintain accurate and confidential records on the time spent on direct service, progress made, and documentation of any unusual circumstances.
• Submit monthly billing reports, payroll time sheets, quarterly reports, accident, serious injury, fall, and medication error reports in a timely manner.
• Attend in‑services as required to improve professional knowledge of concepts associated with intellectual disabilities.
• Assist in dealing with verbally and physically aggressive individuals using agency‑approved techniques to meet treatment needs and human rights regulations.

• High school diploma or equivalent (equivalent education may be considered with proof of knowledge, skills and abilities).
• Ability to understand the special training needs of individuals with intellectual disabilities.
• Current valid driver’s license and good driving record.
• Willingness and ability to travel and use personal transportation in a work capacity.
• Must successfully complete training in Medication Administration, Medication Repackaging, Medicaid Waiver training, CPR, First Aid, behavioral intervention training, etc.
• Ability to lift forty (40) pounds, with or without reasonable accommodation.
• Ability to work scheduled hours, be on time, and follow reporting guidelines for requested time off.
• Willingness to work in other homes, as needed.
• Good written and oral communication skills.
• Effective problem‑solving and decision‑making skills.
• Ability to organize own work and be self‑directed.
• Basic knowledge of community resources.
