• Deliver Physical Therapy Assistant services under supervision for residents with LTAC needs, focusing on mobility, strength, and functional activities.
• Implement and monitor treatment plans created by supervising PT, adjusting as needed based on patient response.
• Facilitate therapeutic exercises, gait and balance training, ROM activities, and functional mobility tasks.
• Document patient progress accurately, communicate results with the supervising PT and the care team, and contribute to plan of care updates.
• Assist with discharge planning, patient education, and caregiver instruction to support continuity of care after transfer or return home.
• Maintain patient safety, proper body mechanics, and adherence to infection control and facility policies.

• Active Physical Therapy Assistant license or credential (as applicable).
• Ability to work effectively in a skilled nursing facility environment.
• Strong communication and teamwork skills to collaborate with the care team.
• Eligibility to work in the United States for a travel assignment.
• Availability for a 13-week assignment with a consistent 8:00 AM–4:30 PM schedule.
