Valley Health - - Responsibilities:
Provide physical therapy services under the supervision of a licensed Physical or Occupational Therapist as part of an interdisciplinary home health team;
Deliver discipline-specific interventions to improve safety and functional independence for patients;
Maintain licensure and participate in ongoing education and competency development;
Complete documentation and adhere to billing and regulatory guidelines in electronic health records;
Communicate patient progression and coordinate care with the supervising therapist and other team members
