Job SummaryUnder general supervision, employee is responsible for providing case management services and crisis intervention to mental health and substance abuse clients. Case management services include hospital discharge planning; assisting clients in identifying employment opportunities, living alternatives, educational, and entitlement opportunities. Employee provides crisis intervention on an on-call basis as assigned by the Director of Clinical Services. Employee acts as a liaison between the Counseling Center and psychiatric institutions (both public and private) and hospitals, relaying appropriate clinical information.
Employee must exercise tact and courtesy in contact with staff, clients, families, public, and service providers. Reports to the MH Care Coordinator for Adults.
- Organizes and mobilizes community resources to meet the needs of clients; provides information regarding employment opportunities, self-help needs, living alternatives, and educational and entitlement opportunities to clients; participates in hospital discharge planning for clients returning to the area.
- Performs a comprehensive assessment of available community resources for clients.
- Conducts pre-admission screening assessment of clients seeking admission to the hospital.
- Acts as liaison between the Counseling Center and psychiatric hospitals (both public and private); relays appropriate clinical information and recommendations.
- Maintains complete, accurate, and current clinical charts and records; performs routine office filing; receives and answers general correspondence and inquiries.
- Develops and maintains liaisons between the Counseling Center and other community agencies and organizations offering aid and services to clients.

- Must pass all required Background Checks.
- Must possess a valid Virginia Driver's License with reliable transportation and driving record acceptable to agency insurance carrier.
- Must obtain a TB test with satisfactory results within 30 days of employment, and yearly thereafter as required by licensing authority.
BA/BS in Human Services, Social Work, Psychology, Sociology or related field, and 1 to 2 years’ experience desired in emergency/crisis services or case management; or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
