The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginians. The VDH is made up of a statewide Central Office in Richmond and 33 local health districts. The Chesterfield Health District provides public health services to the residents of Chesterfield County, Powhatan County, and the City of Colonial Heights.

The Chesterfield Health District is seeking a dynamic and highly motivated Community Health Coordinator to support equitable health outcomes across the District. The Community Health Coordinator plays a key role in leading and assisting community health projects, initiatives, and educational efforts.

Primary activities include coordinating the Community Health Assessment (CHA), Community Health Improvement Plan (CHIP), community vaccination events, and outreach and education initiatives. This position works closely with District residents, coalitions, schools, local leadership, and community organizations to understand and address local health needs. The role also advocates for and drives policy, systems, and environmental change that supports long-term community health improvement.

• Manage short- and long-term project activities related to the CHA and CHIP
• Collaborate with community partners and provide leadership for staff executing day-to-day initiatives
• Facilitate quality and performance improvement using data from the CHA and other sources
• Coordinate resources and support daily operational needs
• Use, interpret, and share data to promote equitable and inclusive access to Health District services
• Advocate for and support policy, systems, and environmental changes aligned with community health priorities
• Assist with preparing grant applications, reports, and related documentation

• Knowledge of Health Education & Promotion:
  Understanding of population health data collection, community demographics, social/cultural environments, and chronic disease prevention. Ability to implement and evaluate health improvement programs and policies for diverse populations.

  Excellent oral and written communication skills to effectively guide staff, educate internal/external stakeholders, and respond to data requests. Ability to develop and deliver training materials, seminars, and health education sessions.

  Knowledge of procurement processes, budget monitoring, and ensuring proper use of resources. Ability to prepare grant applications, manage grant activities, and maintain compliance with funding guidelines.
• Collaboration & Relationship Building:
  Ability to build and maintain effective working relationships with community partners, local health districts, and agencies to promote health initiatives and improve population health.
• Quality Improvement & Data Analysis:
  Skilled in analyzing and verifying data, identifying trends, and preparing reports to support health program planning. Ability to evaluate the success of health programs and make necessary adjustments.
