The Chesterfield Health District of the Virginia Department of Health is seeking a dynamic Community Health Coordinator to coordinate the Community Health Assessment (CHA), CHIP, vaccination events, and outreach efforts to improve equitable health outcomes for Chesterfield County, Powhatan County, and Colonial Heights.
