Salary Range: $16.00 To $17.00 Hourly
We are seeking a compassionate and proactive Patient Advocate to join our team. The ideal candidate will regularly interact with patients to identify their needs, address concerns, and connect them with appropriate resources. This role is critical in ensuring patients receive the support they need, especially within our vulnerable populations.
Key Responsibilities- Greet patients warmly in person or via telephone, providing information on available services, payment categories, and billing procedures.
- Act as a liaison between patients and the office coordinator or operations team to communicate any issues or complaints.
- Assist in coordinating care by facilitating communication with community organizations, health plans, facilities, and specialists.
- Proactively manage the needs of patients and their families, particularly within population health management.
- Effectively communicate with all patients, with special attention to those in vulnerable populations.
- Support providers in helping patients achieve their health goals by connecting them with community resources such as gym memberships, parenting classes, healthy cooking classes, GED programs, and food pantries.
- Cross-train in outreach and provide support as needed or directed.
- Cross-train in front office duties and case management to offer support when required.
- Perform other duties as assigned by the office coordinator or operations team.
- High school diploma or GED required.
- Strong ability and commitment to treating all patients with kindness and respect, even in challenging situations.
- Friendly and approachable demeanor with a passion for working with the public.
- Understanding of community-based organizations and resources.
- Ability to effectively communicate patient concerns to medical staff.
- Dedication to the mission of providing quality healthcare to all individuals, with a focus on serving the underserved.
- Ability to relate to individuals from diverse ethnic, religious, and economic backgrounds.
- Knowledge of and adherence to HIPAA regulations.
