LCSW Case Manager

Kaiser Permanente is seeking an LCSW Case Manager to join our Healthcare & Medical Services team. In this role, you will provide psychosocial assessments, individualized care plans, and ongoing case management for diverse patient populations. You will coordinate interdisciplinary care, facilitate referrals to community resources, support discharge planning, and deliver brief therapeutic interventions. Using data and documentation best practices, you will track outcomes, ensure regulatory compliance, and collaborate closely with physicians, nurses, and other clinicians to improve patient experience and quality of care.

Responsibilities

• Conduct comprehensive psychosocial assessments for assigned patients
• Develop and implement individualized care and discharge plans
• Provide brief therapeutic and crisis interventions as needed
• Coordinate services with physicians, nurses, and allied health professionals
• Facilitate referrals to community and social service resources
• Advocate for patients and families to address barriers to care
• Maintain accurate, timely documentation in the EMRMonitor outcomes and adjust care plans based on patient progress
• Ensure compliance with regulatory, ethical, and organizational standards
• Participate in case conferences, quality initiatives, and team meetings
