Location: Orange

Health Connect America, Inc. seeks a Family Centered Treatment Specialist to provide intensive, in home and community based behavioral health services to children and families. In this role, you will conduct assessments, develop and implement family centered treatment plans, provide individual and family counseling, coordinate care with schools, courts, and community agencies, and document services to meet clinical and regulatory standards.

You will collaborate within a multidisciplinary team, participate in supervision and training, and support crisis intervention, all while promoting safety, stability, and improved functioning for families in their homes and communities.

Responsibilities

  • Deliver intensive in home and community based family therapy and support services.
  • Conduct biopsychosocial assessments and develop individualized family-centered treatment plans.
  • Provide ongoing individual and family counseling focused on safety, stability, and functional improvement.
  • Coordinate services and communicate with schools, courts, and community agencies involved with the family.
  • Respond to and support crisis situations, implementing safety and risk management plans.
  • Maintain timely, accurate clinical documentation in electronic health records to meet agency and payer standards.
  • Participate in multidisciplinary team meetings, case consultations, and clinical supervision.
  • Educate families on coping skills, behavior management, and community resources.
  • Monitor progress toward treatment goals and adjust interventions as needed.
  • Engage families in a culturally responsive, strengths-based, and trauma-informed manner.
