Social Services Director

We are seeking a compassionate, experienced, and dedicated Director of Social Work to lead our social services department and advocate for the emotional, psychosocial, and discharge planning needs of our residents. This leadership role is responsible for ensuring residents and families receive the support, resources, and guidance needed throughout their stay.

• Direct and oversee the facility's Social Services Department.
• Complete psychosocial assessments and develop individualized care plans.
• Assist residents and families with adjustment, crisis intervention, and long-term planning.
• Coordinate discharge planning and transitions of care.
• Facilitate resident and family meetings as needed.
• Ensure compliance with federal, state, and local regulations.
• Collaborate with interdisciplinary teams to promote resident well-being and quality of life.
• Assist residents in accessing community resources, benefits, and support services.
• Maintain accurate documentation and participate in care plan conferences.

• Bachelor's Degree in Social Work, Human Services, or related field required;
  Master's Degree preferred.
• Current Social Work license or certification, if required by state regulations.
• Previous experience in long-term care, skilled nursing, rehabilitation, or healthcare preferred.
• Strong leadership, communication, and organizational skills.
• Knowledge of regulatory requirements and resident rights.
