Human Touch Home Health is seeking a PRN Physical Therapist (PT) to serve patients in Remington, Opal, and nearby communities.
Looking for PRN work without being asked to drive across an entire region? This opportunity allows you to build your caseload around your availability and preferred service area while providing one-on-one therapy in patients' homes.

- Complete physical therapy evaluations in patients' homes
- Develop individualized plans of care
- Provide therapeutic exercise, gait, balance, transfer, and mobility training
- Help patients improve strength and functional independence
- Assess home safety and fall risks
- Educate patients and caregivers
- Monitor progress and adjust treatment plans
- Coordinate care with physicians, nurses, therapists, and other team members
- Complete accurate and timely documentation using Well Sky
- Participate in reassessments and discharge planning
- Active Virginia Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Valid Driver's License and reliable transportation
- Strong clinical assessment and communication skills
- Ability to work independently in patients' homes
- Home Health Experience Preferred, but Not Required
- Well Sky Experience is a Plus
