Patient Services Coordinator

The Patient Services Coordinator is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc., to maintain proper care coordination and continuity of care. The role also assists with day‑to‑day office and staff management.
Responsibilities- Manage schedules for all patients.
- Edit schedules for agents calling in sick, ensuring patients are reassigned timely.
- Update agent unavailability in the worker console.
- Initiate infection‑control forms as needed and submit completed employee infection reports to HRD.
- Serve as backup during lunch hour and other busy times, receiving calls from field staff.
- Assist with weekly case conferences.
- Refer clinical questions to the Branch Director as necessary.
- Maintain the client hospitalization log, entering coordination notes, and send electronic log to all office, field, and sales staff.
- Complete requested schedules as tasks appear on the action screen.
- Ensure staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled with the injection visit.
- Handle all add‑on schedules and applicable orders, including discharge visits, OASIS collections, and recert visits.
- Schedule TIF OASIS collection visits and delete any remaining schedule.
- Reschedule declined or missed visits (if appropriate).
- Process reassigned and rescheduled visits.
- Ensure supervisory visits are scheduled.
- Run all scheduling reports including Agent Summary Report and Missed Visits Done on Paper Report.
- Prepare weekly agent schedules.
- Perform initial review of weekly schedules for productivity and geographic issues, then forward schedules to the Branch Director for approval before distribution to staff.
- Verify visit paper notes in the scheduling console as needed.
- Assist with internal transfer of patients between branch offices.
- If clinical, receive lab reports, assess for normality, fax copies to doctors, make copies for case managers, and route to Medical Records Department.
- Initiate employee/patient infection reports as necessary.
- If clinical, may be required to perform patient visits or participate in on‑call rotations.
- Minimum 1 year of home health experience.
- Prior packet review or quality improvement experience preferred.
- Coding certification is preferred.
Scheduled Weekly
Hours:
40 hours per week.
Pay Range: $40,000 - $52,300 per year (starting base pay; actual pay may vary based on location, experience, etc.).
