Vitality at Home is seeking a Full Time Physical Therapy Assistant (PTA) out of our Virginia Branch. The successful candidate will provide treatment to clients under the supervision of a licensed Physical Therapist. Services include administering therapeutic exercises, modalities, and interventions to improve mobility, relieve pain, and restore function. The PTA documents client responses and collaborates with the Physical Therapist and Branch Manager to ensure effective care delivery.

What You’ll Do- Travel to assigned Minnesota service areas to provide in-home physical therapy services
- Deliver physical therapy treatments under appropriate clinical supervision in accordance with the established plan of care
- Assist clients with therapeutic exercises, functional activities, mobility training, and safe transfer techniques
- Monitor client response to treatment and report progress, changes, or concerns to the supervising clinician
- Measure and document range of motion, functional status, vital signs, and treatment outcomes
- Prepare, operate, and ensure safe use of therapy equipment and modalities
- Educate clients and caregivers on exercises, mobility strategies, and use of assistive devices
- Maintain accurate and timely documentation in compliance with MDH, Medicare, and payer guidelines
- Collaborate with the interdisciplinary care team to support optimal patient outcomes
- Associate’s degree from an accredited Physical Therapy Assistant program
- Minimum of two years preferred, ideally in home health or long-term care
- Valid driver’s license, reliable vehicle, and auto insurance
- Ability to pass MN DHS background study
