Join Vitality at Home as a Physical Therapist and help clients regain strength, mobility, and independence at home!
Vitality at Home is seeking Physical Therapists in Virginia, St. Cloud and Aitkin, MN. Successful candidates will be responsible for delivering skilled physical therapy services in a home health setting. The PT evaluates patient needs, develops individualized care plans, and implements evidence-based therapeutic interventions to improve or restore physical function, mobility, and quality of life. This role includes close collaboration with interdisciplinary team members and educating patients and caregivers on maintaining optimal function and safety at home.

- Graduate of an accredited Physical Therapy Master’s/Doctorate Program.
- Licensed to practice as a Physical Therapist in the State of Minnesota.
- Minimum of one year of experience in physical therapy (home health or geriatric experience preferred).
- CPR certification (BLS) required.
- Valid driver’s license, auto insurance, and reliable transportation.
- Clear MN DHS background study clearance required.
