The Licensed Nurse (LPN/RN) plays a pivotal role in supporting residents within Assisted Living and Memory Care settings. They conduct assessments, devise care plans, manage medications and treatments, implement care strategies, evaluate resident progress, and meticulously document all care services. Utilizing the nursing process, they ensure adherence to the Nurse Practice Act and regulatory standards, working under the guidance of the Resident Care Director while upholding Commonwealth Senior Living's principles.

- Part-Time, 3pm to 11pm;
Every other weekend is required.
- Holds an unencumbered state license as a Registered Nurse or Licensed Practical Nurse.
- CPR and first aid certification, or willingness to obtain.
- Proficiency in medication administration and nursing practices.
- Excellent verbal and written communication skills.
- Ability to grasp complex concepts and make sound decisions under pressure.
- Demonstrated leadership experience, inspiring and motivating others.
- Flexibility to work weekends and adapt schedule for resident care needs.
- Genuine compassion for seniors and their families.
- Proficient in basic office equipment, including personal computers.
- Alignment with Commonwealth Senior Living's mission.
- Minimum one year of nursing experience, preferably in geriatric, psychiatric, or rehabilitation settings.
- Prioritize resident and family satisfaction by treating everyone as a valued customer.
- Ensure compliance with Federal and State policies governing resident care.
- Assist in monitoring and maintaining the Medication Management Program's safety and efficacy.
- Administer medications and treatments accurately, maintaining detailed records.
- Provide direct nursing care and supervise nursing assistants.
- Promptly report changes in residents’ conditions to the Resident Care Director.
- Safely assist residents with mobility and transfers.
- Responsively address inquiries from residents, visitors, and staff.
- Support residents based on their unique care plans.
- Foster effective communication through documentation and daily updates.
- Handle controlled substances securely, reporting any discrepancies.
- Maintain equipment and supplies to ensure resident safety.
- Uphold residents’ rights and promptly report any concerns.
- Adhere to safety and infection control protocols.
- Perform additional tasks as assigned by the Resident Care Director or Assistant Resident Care Director.
- Delegate tasks appropriately while maintaining overall accountability.
- Attend regularly scheduled meetings, conferences, and training sessions to maintain professionallicensure.
- Maintain regular communication with the Resident Care Director.
- Ensure a safe working environment through accident prevention and equipment maintenance.
