Overview
Outpatient dialysis may be viewed as equal parts palliative care and health promotion through live-saving therapy. As an Advanced Practice Provider in dialysis, you will have the opportunity to develop relationships with your patients and provide longitudinal care, without the constraints of 20 minute clinic slots. Our APPs enjoy autonomy and flexibility, working closely within an interdisciplinary team of APPs, physicians, nurses, social work and dieticians, to help patients live their healthiest lives today and achieve their overall health goals.
This role supports high-quality, patient-centered care across multiple dialysis units in the central Virginia region as well as ambulatory CKD clinic care.
Outpatient dialysis rounds consist of weekly patient visits and monthly comprehensive rounds with the interdisciplinary team, collaborating closely with nursing, dieticians, social work, and physicians as well as primary care and other specialists, giving close attention to quality metrics and patient engagement.
The APP will be welcomed into UVA’s Center for Advanced Practice, the professional home for APPs across the health system, and will work under the direct clinical oversight of the UVA School of Medicine’s Division of Nephrology.
Preferred Qualifications- Experience in nephrology and / or dialysis
- Familiarity with hemodialysis and peritoneal dialysis care
- Prior inpatient rounding or outpatient clinic experience
- Comfort working in a collaborative, team-based environment
- Commitment to professional growth and team engagement, including participation in continuing education, quality improvement initiatives, or departmental projects through the Center for Advanced Practice or Nephrology Division
Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen.

- Serve as a clinical leader within the dialysis care team, modeling professionalism, accountability, and patient-centered care.
- Support a collaborative, respectful culture across physicians, APPs, nurses, trainees, and ancillary staff.
- Contribute to the ongoing growth and maturation of the APP team through mentorship, peer support, and role modeling.
- Demonstrate adaptability and leadership presence during periods of high census, operational strain, or clinical complexity.
- Stay up-to-date on current nephrology trends, treatment options, and evidence-based practices.
- Administrative
- Adhere to departmental policies, clinical pathways, and quality and safety standards.
- Participate in departmental initiatives related to quality improvement, patient throughput, documentation excellence, and operational efficiency.
- Support accurate, timely documentation, billing, and compliance with regulatory and institutional requirements.
- Engage in scheduling flexibility to meet the evolving operational needs of the dialysis program
- Clinical
- Assess, diagnose, and treat patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), and those undergoing dialysis.
- Conduct limited physical exams and interpret diagnostic tests to guide treatment decisions including dialysis prescriptions and treatment plans
- Work closely with nephrologists, dialysis technicians, nurses, social workers, and renal dieticians to provide comprehensive care
- Manage multidisciplinary pre-dialysis care in CKD stage 4-5 patients in the ambulatory care setting, including patient education regarding ESRD treatment options
- Monitor and manage dialysis treatments, adjusting prescriptions and modalities as necessary.
- Act as a patient advocate, addressing concerns, providing emotional support, and ensuring patients understand their care options.
- Practice with progressive autonomy at top-of-scope, with appropriate consultation and escalation based on patient acuity and complexity.
