Support Coordinator (Case Manager III)
Department Hiring Salary Range: $54,190 - $60,185
The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Essential Functions- Completes initial and annual assessment of individuals for planning services and supports.
- Develops Individual Support Plan based on assessment information in collaboration with the individual and/or family.
- Provides linkage, coordination, and monitoring of services/supports to include Medicaid Waiver Services provided by private providers, other community agencies, natural support systems and other units in the NCSB.
- Maintains chart documentation that is in compliance with agency guidelines and state regulations. Ensures that documentation substantiates billing for services.
- Conducts face-to-face visits at a minimum of every 90 days and completes monthly activity related to the individual service plan.
- Monitors progress, service delivery, and individual satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated.
- Identifies and accesses resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements.
- Assists individual/family in obtaining needed services on an emergency basis, i.e. hospitalization, housing, food, medications, etc.
- Performs other related duties as assigned.
Bachelor’s degree in a human services field. Two years’ experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
Preferred ExperienceTwo (2) years’ experience in the Intellectual Disabilities field or co-occurring populations preferably in the case management area.
- Valid Driver’s License may be required depending on assignment.
- Certification in basic first aid and CPR, depending on area of assignment.
