Clinical Social Worker - BSW at UVA Health

Join the role as a BSW social worker collaborating with MSW/LCSW social workers to screen and assess patients for medical psychosocial and financial needs, provide generalist social work services within an interdisciplinary team, complete Uniformed Assessment Instrument and other government forms, arrange appropriate services, and educate patients and families about resources.

• Complete initial screening of patients in coverage area per department standards.
• Coordinate with MSW/LCSW and Nurse Case Manager for high-risk and complex patients to ensure continuity of care.
• Explore patient/family current and projected financial resources for healthcare and essential needs.
• Act as a patient advocate.
• Perform additional duties as assigned.

• Education:
  
  Bachelor’s degree in Social Work from a CSWE accredited program.
• Experience:
  
  No experience required.
• Licensure:
  None.

Job requires sitting for prolonged periods, standing/traveling, use of assistive and climbing, proficient communicative, auditory and visual skills; attention to detail; ability to lift/push/pull < 20lbs. May be exposed to chemicals, blood/body fluids and infectious disease.

Hourly range: $24.24 - $38.78.
