Mental Health Professional Residential I Sub-Relief

The most essential role of the Mental Health Professional Residential (MHP) is to contribute to the best life possible for individuals with serious mental illness, substance abuse or developmental disabilities. The position provides skill building and support to promote maximum independence for each individual providing guidance and safety support in all areas ensuring that they are active and valued members of their community.

The MHP provides guidance to support staff as designated by and in the absences of the Manager Mental Health Group Home; providing training and support of individuals support activities and plans. Assist supervisor as needed regarding required aspects of service delivery including, but not limited to, development and implementation of individual service plans and documentation of services delivered.

ESSENTIAL FUNCTIONS
  • Provide the best life possible for all individuals served in each aspect of their lives by providing skill building and supports to promote maximum independence for each individual according to their specific needs in accordance with their Person-Centered Plan. Provide training, skill building and support to include personal care, socialization, community integration, educational activities, volunteer opportunities and other adaptive living skills.
  • Assist supervisor as needed regarding required aspects of service delivery including, but not limited to, development and implementation of individual service plans, training staff on ISPs, writing person centered reviews of services, and ensure services are delivered per the approved plans and schedules. Provide leadership support regarding questions and concerns during service hours. Review plans and quarterlies for changes or recommendations as identified including but not limited to protocols and SIS assessments.
  • Effectively intervene in crises and emergencies, to include administration of first aid, following Agency policies and procedures and seeking consultation from supervisor and other resources as appropriate. Be knowledgeable of and provide support for medical needs while with the individual, which includes helping the individual explain their symptoms when sick and to understand and follow doctor s orders while providing care for the individual in service.
  • Responsible for accurate, informative documentation reflecting the services and supports delivered to the individuals served. Complete daily documentation of progress notes, medication administration, time sheets, and other reports in an accurate, timely, and professional manner. Must manage time independently and ensure meeting billing and documentation expectations.
  • Successfully complete the Board of Nursing approved initial medication class and maintain an annual recertification. Responsible to administer medications as prescribed, monitor for any potential side-effects, reporting observed information to the prescribing physician and documenting per Agency procedure.
  • Ensure all required trainings are completed in compliance with Agency policy. Responsible to maintain current certification in First Aid/CPR/AED and other trainings as required or assigned.
  • Must be able to work independently and as part of a team to actively participate in daily activities for the individuals served. Must be able to work without direct supervision. Attend scheduled meetings and provide information that informs the development of a person-centered service plan to support individuals served in the community. Maintain data collection methods and update supervisor regularly regarding progress.
  • As required, transport individuals using Agency vehicles.
OTHER DUTIES
  • Participate in meetings held for a variety of purposes including reviews for persons served by (but not limited to), residential services, administration and in-service training.
  • Participate in and actively promote safe practices in the delivery of services.
  • Other duties assigned by supervisor including communication with individual’s families and support team.
