Advanced Practice Provider (APP) – Transplant (Solid Organ Transplant Recipients and Living Donors)
The Transplant APP provides comprehensive care to liver, kidney, and pancreas transplant recipients and living donors. Working both independently and collaboratively with physicians and an interdisciplinary team, the APP performs detailed assessments, formulates diagnoses, orders and interprets diagnostic studies, and develops and manages treatment plans. Responsibilities include prescribing therapies, initiating specialty consultations, managing acute and emergent clinical situations, and facilitating efficient patient flow.
The APP communicates care plans effectively with patients and families, provides specialty consultation, supports clinical pathways and protocols, and ensures patient advocacy, education, confidentiality, and high-quality, patient-centered care. This is a 24/7 position and a nocturnal option is available.

The APP practices independently and collaboratively with transplant physicians/surgeons and interdisciplinary team members to assess, diagnose, treat, and manage complex transplant patients while supporting clinical operations, patient education, and interdisciplinary collaboration.

The APP practices independently and collaboratively with transplant physicians/surgeons and interdisciplinary team members to assess, diagnose, treat, and manage complex transplant patients while supporting clinical operations, patient education, and interdisciplinary collaboration.
Administrative Responsibilities- Ensures accurate, timely, and compliant documentation in the electronic medical record
- Maintains confidentiality of patient information in accordance with institutional, state, and federal regulations
- Facilitates timely patient flow through effective coordination with interdisciplinary team members
- Supports clinical operations through adherence to institutional policies, procedures, and clinical pathways
- Participates in quality improvement initiatives, clinical audits, and workflow optimization as assigned
- Practices in accordance with professional standards, regulatory requirements, and scope of practice for Advanced Practice Providers
- Communicates effectively with patients, families, physicians, nursing staff, and interdisciplinary team members
- Advocates for patients and families regarding educational needs, treatment options, ethical considerations, and patient rights
- Demonstrates cultural competence and sensitivity to the diverse needs of transplant patients and families
- Engages in ongoing professional development, continuing education, and maintenance of certification and licensure
- Provides comprehensive clinical care to transplant patients independently and in collaboration with transplant physicians and interdisciplinary team members
- Performs complete and focused histories and physical examinations
- Documents clinical findings, including pertinent normal and abnormal findings, in the medical record
- Orders, interprets, and evaluates diagnostic laboratory tests and imaging studies
- Formulates medical diagnoses based on clinical assessment and diagnostic findings
- Develops, implements, and evaluates individualized treatment plans
- Prescribes and recommends pharmacologic and non-pharmacologic therapies within scope of practice
