As a Dementia Support Co Ordinator within the Calderdale Dementia Hub, you’ll play a vital role in delivering a dynamic and innovative Dementia Information and Advice Service across Calderdale and Kirklees. You’ll support individuals at every stage of their dementia journey — alongside their families and carers — providing personalised guidance, practical advice, and tailored support plans that empower people to live well and confidently.

In this rewarding role, you’ll manage a varied caseload, bringing a compassionate, person centred approach to every interaction. You’ll organise community appointments and telephone consultations, co produce meaningful personal plans and help individuals maintain independence, reduce isolation, and navigate challenges with dignity.
