The Southern Touch Home Care is seeking an outgoing, motivated, and relationship-focused Community Care Liaison to join our growing healthcare team.

This is a healthcare community outreach position focused on building relationships with local healthcare providers and community organizations while helping families learn about available in-home care resources. This is not a cold-calling or retail sales position. It's an opportunity to educate, connect, and make a meaningful difference in the lives of children and families in our community.

• Building relationships with physicians' offices, hospitals, rehabilitation facilities, therapy providers, schools, and community organizations.
• Educating healthcare professionals and families about our home care and GAPP services.
• Representing The Southern Touch Home Care at community events and networking opportunities.
• Identifying opportunities to connect families with the resources and services they need.
• Maintaining a positive, professional presence throughout the community.

• Friendly, confident, and enjoys building relationships.
• Self-motivated and able to work independently.
• Organized and dependable.
• Comfortable traveling throughout the local community.
• Passionate about serving others and making a positive impact in healthcare.
• Part-time to start.
• Potential to grow into a full-time position as the role expands.
• Experience in healthcare, community outreach, marketing, or customer relations is preferred, but we are willing to train the right candidate.
