AboutThe Role
We’re seeking a compassionate Registered Nurse (RN) to join our hospice team—available full‑time, part‑time, or PRN. As an RN Case Manager, you’ll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families. You’ll collaborate with an interdisciplinary team to ensure every patient is cared for—body, mind, and spirit.
How You’ll Make a Difference- Assess patient and family needs and develop personalized care plans.
- Deliver skilled, compassionate nursing care and symptom management.
- Administer medications and coordinate care across the healthcare team.
- Educate and support families to promote comfort and peace of mind.
- Lead and guide hospice aides to ensure consistent, compassionate care.
- Maintain accurate documentation and participate in ongoing education.
- Current RN license in Virginia (multi‑state or single‑state).
- CPR certification and at least two years of nursing experience (hospice or home health preferred).
- Strong clinical judgment, communication, and organizational skills.
- Reliable transportation, valid driver’s license, and insurance.
- A self‑directed, compassionate spirit ready to serve your community.
