Responsibilities- The Nurse Liaison ensures timely and seamless transition of infusion patients from hospital to home and serves as the first point of contact for new home health referrals.
- Coordinate clinical activities, review documentation to ensure appropriateness and readiness for home infusion services, and facilitate effective communication among care teams.
- Collaborate with physicians, hospital discharge planners, and interdisciplinary team members to develop individualized plans of care.
- Provide education to patients and caregivers to ensure confidence and support in managing IV care at home.
- Adhere to professional practice standards and integrate the ANA Nursing Scope and Standards of Practice with the UVA Nursing Professional Practice Model.
- Other duties may be assigned as needed.
- Education:
Graduate of an accredited nursing program;
Bachelor of Science in Nursing required within 5 years of hire. - Experience:
1 year of relevant experience. - Licensure:
Licensed to practice as a Registered Nurse in the Commonwealth of Virginia; valid Virginia Driver’s License;
American Heart Association (AHA) Health Care Provider BLS certification. - Physical Demands:
May require sitting for prolonged periods, standing, traveling, bending/stooping; ability to lift/push/pull 20–50 lbs; proficient communication, auditory and visual skills; ability to write legibly; exposure to chemicals, blood/body fluids, and infectious disease as applicable.

Starting base rate for this role is $82,992.00 annually. Individual compensation will be determined by the selected candidate’s qualifications, previous work experience, and/or education.
