Position Details
- Location:
Norfolk, VA - Schedule:
Monday-Friday, 8:00 AM - 5:00 PM - Contract Length: 13 Weeks
- Patient Load:
Approximately 5-8 home visits per day
- Minimum 1 year of recent Home Health (or Hospice) experience
- Active VA LPN/LVN License
- BLS/CPR Certification
- Recent Home Care Home Base (HCHB) experience
- OASIS experience preferred
- Reliable transportation and willingness to travel within the assigned territory
