Complex Care Manager RN - Tidewater VA Markets

About This Role

The Complex Care Manager, RN will manage complex members, facilitating and advocating for their ongoing self-sufficiency and independence. This role is ideal for those who thrive in less structured environments and are passionate about coordinating care for high-risk patients.

• Location: Williamsburg, VA
• Schedule: Full Time, Day shift
• Residency: Must reside within Tidewater, VA Markets commutable to Williamsburg, Virginia Beach, and Norfolk, VA
• Work Style: Flexibility to work remotely for those within a commutable distance

• Engage members/families telephonically and/or face to face to coordinate services, community resources, and treatment needs
• Deliver a holistic approach to coordinated care based on the member's needs using a person-centered philosophy
• Identify early risk factors and conduct ongoing assessments, documenting in an electronic file
• Create, review, and revise care plans focusing on a holistic approach
• Function as an advocate for members
• Collaborate the members' PCP to deliver and coordinate necessary services
• Build relationships with members and their families; assist them with coordination of health choices
• Ensure cases are documented in a timely manner
• Coordinate with other state agencies, community resources and providers
• Act as the primary point of contact for members and the Interdisciplinary Care Team (ICT)

• Education: Current, unrestricted RN licensure in the state of Virginia
• Experience: 2+ years of recent experience in case management, home care, long-term care, and/or experience in acute or rehab care setting
• Experience: 1+ years of experience directly working with individuals with complex medical or behavioral needs

• Hourly pay range: $29 - $52 per hour based on full-time employment
