Clinical Care Manager

LMK Recruiting is seeking dedicated individuals who are passionate about making a difference in the lives of older adults. As a Clinical Care Manager at, you will play a crucial role in ensuring that our clients receive the highest quality of support.

• Comprehensive Care Management:
  Oversee and coordinate tailored care plans, ensuring that our clients receive appropriate and timely services.
• Client Advocacy:
  Serve as a strong advocate for clients, ensuring their needs are met in home, medical and care facility settings, including while communicating with healthcare providers.

  Work closely with team members to address clients’ needs, and contribute your expertise to the team, helping to enhance the collective knowledge and skills.
• On-Call Availability:
  Participate in after-hours or weekend on-call rotations as needed to provide continuous support to clients.
• Field Work and Documentation:
  Update family members and other members of the care team, and maintain thorough and accurate documentation. Approximately 90% of the role involves field work, with 10% dedicated to follow-up and office-based tasks.

• A master’s degree in a related field, or a bachelor’s degree with at least 4 years of relevant experience in gerontology, mental health, home care, or similar.
• Available 9am to 6pm Monday-Friday and some weekends
• Dependable, adaptable, and accountable, with a strong commitment to providing exceptional care and support to clients.
• Must be able to lift and maneuver up to 50 pounds.
• Must be comfortable and willing to drive regularly around the Virginia and DC Metro area.

Ideal candidates can be from a variety of backgrounds including RN, LPN, OT, PT, and SLP

• Hourly, non-exempt
