The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities across multiple participants, using advanced critical thinking to develop, implement, and evaluate individualized care plans that empower patients and optimize outcomes. The RNCC marshals and coordinates resources and patient care activities to support transition management, collaborating across the continuum to ensure individualized follow-up and coordination of services across health care settings and locations.

The Role

This is a dynamic, patient-centered position grounded in both clinical coordination and education. You will serve as a critical link between patients, families, and providers, helping guide care across both outpatient and inpatient settings while ensuring patients feel confident managing chronic conditions.

In this role, your day-to-day experience includes both structured teaching and clinical coordination responsibilities within a supportive pediatric specialty team.

Responsibilities
  • Coordinate care for pediatric patients with diabetes and endocrine disorders across clinic and hospital settings
  • Provide in-depth patient and family education, including new diagnosis diabetes education, insulin pump training and ongoing disease management support, teaching in both classroom-style settings and at the bedside for admitted patients
  • Manage phone triage and patient communication, supporting families as a consistent clinical resource
  • Collaborate closely with physicians, advanced practice providers, and fellow RN Care Coordinators in a highly integrated team model
  • Participate in a rotating Saturday call schedule shared among the RNCC team
  • Progress toward independent teaching, clinical decision-making, and triage management within the first year

This role is primarily clinic-based and in-person
, offering consistency in schedule while maintaining meaningful patient interaction throughout the day. The position is part-time, with a preference for candidates available closer to 24 hours per week. The pace is steady, purposeful, and centered on building strong patient relationships and delivering high-quality education.

Relocation assistance may be available for qualified candidates.

Physical Demands

The job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20–50 lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.

The pay range for this role is $41.26–$54.52 hourly. Individual compensation will be determined by the selected candidate’s qualifications, previous work experience, and/or education.

Minimum Requirements

Education: Bachelor of Science in Nursing from an accredited nursing program.

Experience: 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.

Licensure/Certification: Licensed to practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required. Specialty Board Certification strongly preferred.
