National leaders in care, Gilchrist and Inova have joined to form a new hospice organization offering services throughout Northern Virginia.

The Hospice RN Weekender reports to the Team Manager and is responsible for identifying patient/family care needs, providing nursing and supportive care for terminally ill patients and their families, and coordinating with interdisciplinary teams.

Education

Diploma, ADN and/or BSN from an accredited institution.

Licensures / Certifications

Current Virginia State license as a Registered Nurse.

Experience

One to two years of nursing experience. Home Health or Hospice experience preferred.

  • Knowledgeable of universal precautions for infection control and practice safe nursing standards for self‑protection.

  • Excellent interpersonal and communication skills.
  • Ability to be patient and family‑focused.
  • Skill in the use of EMR (Electronic Medical Record) and/or ability to learn.
  • Skill in the use of portable computer or mobile device.
Physical Requirements
  • Ability to stand and walk almost constantly up to ninety‑percent of work time and lift and position patients up to fifty‑percent of work time.
  • Ability to perform home care visits.
  • Ability to multitask.
  • Ability to concentrate, pay close attention to detail, and function effectively in stressful situations.
Working Conditions
  • Normal office environment with driving as a large part of the day in the field, which includes visits to patient homes, some in unsafe areas (escort provided).
  • Exposure to adverse working conditions due to patient care activities and exposure to communicable diseases.
  • Visits may be in a home, facility, Group Home, Assisted Living Facility, or Nursing Home.
Conditions of Employment

Reliable transportation and auto insurance. Valid Driver’s License.

Principal Duties and Responsibilities
  • Complete initial assessment of patient and family to ascertain home care needs—including physical, emotional, spiritual, and mental status.
  • Prepare and implement the care plan for the patient/family unit in cooperation with the Interdisciplinary Group.
  • Provide direct nursing care to patients to maintain the highest level of care in conjunction with the primary physician.
  • Cooperate with Clinical Team Leader in utilization of other team members, outside agencies, and community resources.
  • Update the primary physician as necessary and regularly on a monthly basis.
  • Update the interdisciplinary team at patient care conferences.
  • Teach family members or hired caregivers patient care skills, including pain control, medication titration (within prescribed range), injections, blood draws, IV management, dressing changes, hygiene, nutrition, checking vital signs, and addressing emotional needs.
  • Ensure arrangements for equipment necessary in the home setting are made available.
  • Attend to patient and family at the time of patient’s death.
