Registered Nurse Case Manager - Field in Danville & Martinsville, VA

About This Role

The Registered Nurse Case Manager will support members in the Danville & Martinsville, Virginia area by delivering coordinated, patient-centered care through in-person visits and telephonic outreach. This role is ideal for compassionate and motivated individuals looking to improve health outcomes for their community.

• Pay: $60,522.00 – $
• Location: Richmond, VA
• Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST

• Conduct comprehensive assessments (telephonic and face-to-face) to evaluate members’ medical, behavioral, and social needs.
• Develop and implement personalized care plans that promote both short- and long-term wellness.
• Coordinate care across providers, programs, and services to ensure a seamless experience for members.
• Use clinical expertise and data insights to identify risks, gaps in care, and opportunities for intervention.
• Support members with complex or multiple conditions by applying a holistic, whole-person approach.
• Facilitate referrals to appropriate clinical and community resources.
• Participate in case conferences and collaborate with multidisciplinary teams to optimize care strategies.
• Review claims history and benefit eligibility to inform care planning.
• Engage members using strong interviewing skills to build trust and encourage active participation in their care.
• Ensure all case management activities comply with regulatory and organizational guidelines.

• Associate Degree in Nursing (required)
• Bachelor of Science in Nursing (BSN) (preferred)

• At least 3 years of clinical experience (hospital, home health, ambulatory care, or similar settings)

• Active, unrestricted RN license in Virginia
