What You'll DoAs an RN Case Manager
, you'll be the heart of the care team, serving as a trusted advocate for patients and families while coordinating personalized hospice services.
- Assess patient and family needs and develop individualized care plans
- Deliver skilled nursing care focused on comfort, symptom management, and quality of life
- Coordinate care among physicians, hospice aides, social workers, chaplains, and other team members
- Educate and support families through every stage of the hospice journey
- Supervise and guide hospice aides to ensure exceptional patient care
- Maintain accurate clinical documentation and participate in ongoing professional development
We're looking for a nurse who combines strong clinical skills with a servant's heart.
Qualifications:- Current Virginia RN license (single-state or multistate)
- CPR certification
- Minimum two years of nursing experience
- Hospice, palliative care, or home health experience preferred
- Strong communication, assessment, and organizational skills
- Valid driver's license, reliable transportation, and automobile insurance
- A compassionate, self-motivated approach to patient-centered care
