Hiram W. Davis Medical Center, located in Petersburg, Virginia, is a medical facility providing acute medical diagnoses, skilled care, intermediate care, and long-term care to individuals receiving services in DBHDS facilities and within the community that have intellectual and/or developmental disabilities. Admissions to HWDMC are generally either permanent placement or special hospitalization. Patients typically come to HWDMC as a special hospitalization for step-down to stabilize, post-surgery rehabilitation, or long-term care.

• Monitor and document patient health, behavior, and treatments and reporting any changes to licensed staff.
• Support one-to-one supervision, safe patient transport, and maintain a clean, safe environment.
• Coordinate staffing, participate in therapeutic activities, and ensure care is delivered according to hospital policies and standards.

• Knowledgeable in practical nursing principles, patient care, anatomy, pharmacology, and hospital safety.
• Skilled in medication administration, patient assessments, IV/PICC care, catheterizations, and restorative exercises.
• Able to implement care plans, supervise staff, provide one-to-one patient care, and respond to emergencies.
• Proficient in documentation, communication, and coordinating patient transport and behavioral plans, with flexibility to work varied shifts and overtime.
• LPN's will possibly work two 12 hour shifts and two 8 hour shifts per week or a schedule to equal 40 hours.

• Current licensure as LPN with the Virginia State Board of Nursing or equivalent compact license.
• Graduation from an accredited School of Practical Nursing along with successful completion of an acceptable Pharmacology course.
• Knowledge of medication administration and EHR systems.

Experience working in a long-term care setting preferred.
